Under the Grade "A" Dairy Assessment Act, a dairy producer may request a full or partial refund of the assessment collected.
You can either download the assessment refund form here and return it via email or US mail, OR complete the form online here.
Please complete the form and provide proof of payment of the assessment to the Association no earlier than December 15 and no later than December 31 of a calendar year.
Acceptable documentation for proof of payment may include:
Processor or handler statements.
Other satisfactory proof of assessment deduction or payment, such as a letter from a milk cooperative.
The NC Dairy Producers Association shall mail a refund to the dairy producer within 120 days of receipt of a properly completed and documented refund form.
